Long-Term Kidney Function Concerns:

Recent research suggests a potential increased risk of kidney problems with long-term use of ACE inhibitors, particularly in patients with pre-existing kidney issues. For instance, a study from the University of Virginia School of Medicine raised concerns that long-term use of ACE inhibitors could be contributing to kidney damage. The researchers found that long-term use of drugs that inhibit the renin-angiotensin system, such as ACE inhibitors, was associated with hardened kidney vessels in both lab mice and humans. However, the researchers stressed the importance of continuing to take these medications, as they can be lifesaving for patients, and called for additional studies to better understand the drugs’ long-term effects on the kidneys. In another study, it was found that ACE inhibitor use does not appear to decrease the long-term risk of end-stage renal failure in diabetes. The data suggested instead that ACE inhibitors might actually increase this risk. However, it’s important to note that ACE inhibitors can still benefit people with kidney disease or heart failure, even if they do not have high blood pressure. If high blood pressure is not present, a lower dose may be recommended to prevent low blood pressure symptoms.

Cost-Effectiveness Considerations:

While the availability of generic ACE inhibitors has improved affordability, healthcare systems and payers are increasingly cost-conscious. For instance, many ACE inhibitors are available as lower-cost generics. You can find lisinopril, enalapril, and benazepril for less than $10 at certain pharmacies with a GoodRx discount. All of them are taken by mouth and most are available as low-cost generic options. Most are taken once a day, although some may need to be taken more often (2-3 times per day). In terms of cost-effectiveness, a study found that prescribing ACE inhibitors as antihypertensive first-line therapy in patients without cardiovascular morbidity cannot be recommended at the present time unless the acquisition costs of ACE inhibitors become substantially more attractive. The incremental cost-effectiveness of prescribing ACE inhibitor therapy to everybody was never less than $US100,000 /QALY in the sensitivity analysis. Another study found that ACE inhibitors are a cost-saving treatment compared with placebo in non-diabetic nephropathy, irrespective of ACE (I/D) genotype. However, ACE inhibitors saved more costs and more health gains were achieved in the ACE DD genotype than in the ACE II/ID genotype. Manufacturers need to demonstrate the cost-effectiveness of ACE inhibitors compared to newer medications.

The between-group differences for major secondary outcomes were −1.47 (p = 0.18) in 24-h diastolic BP, −2.86 (p = 0.13) and −2.74 (p = 0.03) in daytime systolic and diastolic BP, and −0.45 (p = 0.82) and −0.93 (p = 0.44) in nighttime systolic and diastolic BP.
